Due to some changes in the way Frevvo handles user identities on forms, we have issues with our Theme Tool when the user IDs are hidden.
The temporary work around is to use the username of whoever owns the form (ie: "nursing") in the URL.

We replaced the alphanumeric identity between "user=" and "&app=" to "nursing" and it resolves the issue. The new link is below:
